Body horror cinema has generated extensive discourse about the psychological impact of extreme visual concepts. One analysis of shock value and transgression noted that filmmakers continually escalate the grotesque in pursuit of audience disturbance.

But one early 2000s film reached an apparent saturation point, a boundary beyond which escalation seemed impossible. A particular horror concept—the human centipede—prompted a retrospective analysis suggesting it represented the ultimate transgressive image. Yet interviews with creators hinted at older, darker concepts they had considered but rejected as unmarketable. One producer suggested that certain ideas about human modification existed so far outside acceptable discourse that even horror cinema wouldn't explore them. The implication suggested a martial artist had generated or embodied ideas about physical transformation and modification so absolute that the film industry, dedicated to transgression, found them unsuitable even for horror audiences. What horror couldn't monetize became relegated to rumor and speculation.
